- Demonstrate Character/Leadership/Community Service/Citizenship
- MHS Alumnus for 20 years
- NJSIAA State Championship
- Mountain Valley Conference Championship/Middlesex County Championship
- Final # 1 ranking in the State, County, or Local Media Area
- Undefeated Season
- Relay Teams – County or State Championships
- Must be 10 years after initial team accomplishment
- Dedicated Service to MHS
- Win/Loss Record @ MHS
- Winning Percentage
- Coached a Championship Team
- Coach of the Year Honors
- State and/or Hall of Fame Honors (NJSCA)
- Demonstrates Character/Community Service/Citizenship
- Coached All-State Athletes
- Must be a Retired Head Coach for 10 years
- Extraordinary Dedication of Time and/or Resources to MHS Athletics
- Distinguished Length of Service to MHS Athletics
- Exemplary Coaching Contributions to the Varsity Program
- Demonstrates Professional Conduct and Character
- Distinguished themselves at the professional/collegiate level as a coach, official, or administrator
- Assisted Head Coach in Multiple Championships
